Before you begin

Your clinician has sent you these questionnaires as part of an autism assessment. Please read through the following before you start.

A note before you start. Some of these forms are long, and a few questions may feel repetitive or hard to answer. This is intentional. The overlap helps your clinician build a fuller picture across different areas of your life.

Please answer as honestly as you can. If a question is unclear, doesn't apply, or you simply don't know the answer, it's fine to leave it blank or skip past it. A partially completed form is still useful. Please submit whatever you can.

A detailed questionnaire covering how you relate to others, process language, perceive your senses, and experience daily life, across both childhood and the present.

Informant form: to be completed by a family member or close contact

Completed by someone who knew you in childhood, ideally a parent, sibling, or other close family member. They rate your behaviour and development based on what they observed, both in your early years and more recently.
